I hereby declare that I have read and understood the KVKK Disclosure Statement for Job Applicants prepared by PROJX Digital İnovatif Teknolojiler A.Ş. ("PROJX") in accordance with the Personal Data Protection Law No. 6698 ("KVKK").

I give my explicit consent for my personal data to be processed and, where necessary, to be stored in electronic/physical environments for the purpose(s) I have marked below:

Within the scope of assessing my suitability for the position and of planning occupational health and safety measures, I give my explicit consent to PROJX processing my medical report / fitness-for-work information.

☐ Transfer abroad

Within the recruitment evaluation processes, in the context of processes carried out via PROJX's business partners / group companies / affiliates located abroad, or through HR and infrastructure systems located abroad, I give my explicit consent to the transfer of my personal data shared during the application process abroad, within the conditions stipulated by the KVKK.
